Question 1/7: Do you know someone who has a disability?
	Yes, I have a disability.
	A family member or close friend does.
	An acquaintance or co-worker does.
	Not really

Question 2/7: How do you think most adults with disabilities live?
	In a hospital or institution of some sort.
	At home by themselves.
	With their families.
	With a roommate.

Question 3/7: When the parents of someone with a disability die, what do you think happens to that person?
	A friend or family member usually lets them move in.
	An institution takes them in.
	They have to figure out how to live on their own.
	The disabled person usually dies first.

Question 4/7: Who do you think has the most influence on people with disabilities?
	Family.
	Friends.
	Service providers.
	Teachers.

Question 5/7: What best describes your attitude towards people with disabilities?
	A strong, healthy society is inclusive.
	Resources should first go to the people who need them most.
	I know it’s not popular to say, but I don’t think people with disabilities are the same as the rest of us in society.
	I haven’t thought much about it yet.

	Becoming independent.
	Being creative and expressing myself.
	Making connections with people.
	Money and stability.

	What happens when I’m gone?
	Will my child live a full life?
	How can my child move out and still be safe?
	How can I get real, lasting help?
